Egg Donation Procedure - The Donor Egg Program for South African Egg Donors
Screening for egg donation - this happens once an egg donor is required to participate on an egg donor program:
-
Consultation/ Counselling - Egg donors are encouraged to ask as many questions as you need to feel comfortable. - Have a meeting to sign consent forms, etc. The process of egg donation and every thing involved in egg donation will be explained again. Egg donation is an important aspect and all persons participating as egg donors should have the opportunity to have a private meeting to be counselled and ask any thing that they need to know.
-
Blood tests - testing for HIV, blood type (if not known), prolactib, thyroid, function, fasting insulin and fertility hormonal tests. Normally the egg donor will be required to fast (no eating and no drinking, except water) for 10 hours prior to the tests.
-
Scan - This is an internal scan performed by a fertility specialist to verify that the reproductive organs are in tact and that that ovaries appear normal.
Please confirm the exact screening for the egg donor program - which is specific to the fertility clinic.
Egg Donation Process for Egg Donors: Once the screening confirms the egg donor's suitability, she may be given the contraceptive pill by the ferility sister as part of the process to start synchronising her menstrual cycle with the recipient.
The actual egg donation treatment or egg donor program commences when the egg donor starts her period. With the start of the egg donor programme, the egg donor takes medication to stimulate follicle and egg development for a period of about 2 weeks. The donor will need to have injections over a period of 2 to 3 weeks. The injections are very easy to administer and are administered much the same as a diabetic would give himself an insulin injection.
During this time of about 2 weeks – the egg donor comes in for about 2 or 3 scans to monitor the donors’ response to the medications and to evaluate egg size and growth. These scans have to be done on specific days but egg donors can come in to the clinic from as early as 7am and donors are not required to wait. Egg donors are seen first because every one knows what a special deed they are doing. These scans are really quick, so you can be in and out in no time. These scans are normally on day 2 and day 10.
Once the egg donor ovulates, the eggs are ready to be extracted. This is a very simple procedure that requires a morning at the clinic. The actual egg retrieval process differs per clinic. Egg donors may be administered full anaesthetic to let them sleep or a mild anaesthetic when they ar awake during the process. Please confirm this with the fertility cilinic. The donors' eggs are retrieved via the vagina. This is not an operation – there are no incisions that are going to hurt afterwards or leave any scars. Worst side effect is slight bloatedness. Egg donors can go home within an hour or two of having the procedure done.
Egg Donation Remuneration in South Africa - This is paid by egg donation recipients on the day of egg retrieval - when the donor eggs are ready to be donated:
-
Egg donors are not responsible for any of the costs that are attached to this procedure. The only responsibility on the egg donor is to get to the clinic and arrive for the scheduled appointments.
-
Egg donors will receive a cash donation of between R5,000 and R6000,00 on the day of egg retrieval - dependent on the fertility clinic. This is usually paid left with the fertility sister or Accounts Department. Please confirm the exact process and amount when you start at the fertility clinic.
-
In the unfortunate event that the egg donor program is cancelled because the egg donor has not responded well to egg retrieval, she is likely to receive less. This is also clinic dependent.
-
In the unfortunate event that no eggs are retrieved on the day of egg retreival, the egg donor may receive less for her egg donor help. Please check the conditions with the fertility clinic when you proceed.
Egg Donor Confidentiality
Egg donation is kept anonymous and confidential. Egg donors are treated with absolute respect and their details kept entirely confidential. Upon being interested in egg donation, you may be asked for a picture of yourself. This is just to match you up to a recipient. If you are uncomfortable with this, it is quite acceptable and you need not provide any picture. Egg donation is a generous deed of the heart to help another couple. The egg donor may know the profile of the couple she is helping. Similarly the donor’s profile will be communicated to the recipient. There is never an exchange of names. South African Egg Donation encourages anonymous egg donation and confidential egg donation.
Risks Iro Egg Donation
-
The egg retrieval process is not dangerous.
-
The medication does have slight side effects in that it makes you a little irritable, sometimes tearful. One can feel bloated almost as if you are about to have a period. Some donors seem to breeze through with very little side affects, others seem to feel it more. Drinking a lot of water helps to minimise any discomfort. The egg retrieval process may make egg donors feel a little bloated and uncomfortable for a few days, but as each day goes by it will get better and by the time your next menses arrive you should be back to normal again. This should happen 10 days to 2 weeks after you have had the eggs removed.
-
Egg donation is not harmful to your own fertility. Neither do any of the drugs have a lasting side effect nor do they do any damage to your ovaries. In fact nothing that we do will compromise your fertility, however you need to understand there are other underlying reasons for infertility; for example blocked tubes, poor sperm count, and we could never assure you that later on you may not have a problem with something like that as we are only focused on your ovaries at this point.
Egg Donation Frequently Asked Questions / Egg Donation FAQ's/ Egg Donor FAQ/ Egg Donor Questions
Q: Is egg donation legal? A: Yes, definitely.
Q: Do I get paid to be an egg donor?
A: Egg donors do not get paid, they receive a donation. Please see the section above.
Q: Why do ladies or couples need egg donors? A: Refer to the section on why couples need egg donors/ why they need to participate on an egg donor programme.
Q: Do ladies donate again after they have done it? A: Yes, egg donors find the process very rewarding and so wish to help out again. They have found the discomfort minimal. Every egg donor I have encountered has offered to donate again.
Q: How often can ladies donate their eggs? A: Egg donation is governed ito the Human Tissue Act. An egg donor can donate her eggs to the extent that 5 children are conceived through egg donation. Egg donation is also governed by the National Health Act.
Q: How often after I have donated can I donate again? A: After the egg donation, you will have a period, then you need to have a normal period and can donate again in the next cycle.
Q: Is it okay if I donate eggs to get the egg donation money? A: Egg donors are angels, so the recipient gladly offers the R5,000 or R6,000 for the egg donor’s inconvenience.
Q: How do I know that my eggs are not used for testing and/ or given to couples who will nor adequately look after the child? A: The fertility clinic who will do the egg retrieval process is absolutely professional and rated as the top fertility clinic in Johannesburg. Couples receive counselling before embarking on the egg donor programme. The clinic does not engage in any testing at all.
Q: What happens to the eggs that are not used? A: After the eggs have been retrieved, they are fertilised with the recipient’s partner’s sperm. They are grown in a special medium for 5 days, at which stage usually the best two are implanted into the recipient’s uterus. If there are remaining embryos they continue to grow in the special medium for another day or two, and are then frozen – if viable.
Q: Does the egg donation recipient know who the egg donor is? A: Egg donation is kept anonymous and confidential. Egg donors are treated with absolute respect and their details kept entirely confidential. Upon being interested in egg donation, you may be asked for a picture of yourself. This is just to match you up to a recipient. If you are uncomfortable with this, it is quite acceptable and you need not provide any picture.
Q: Can I be an egg donor if I am a virgin? A: Yes. The egg donor must be prepared to have an internal examination. Some South African fertility clinics are however not comfortable to do treatment on virgin egg donors.
Q: Can I be an egg donor if I am a lesbian? A: Yes, the egg donor’s sexual preference is her own decision and does not impact on her ability to be an egg donor.
Q: Can I be an egg donor if I am breastfeeding? A: Egg donors first have to finish breastfeeding and can then donate.
Q: Can I be an egg donor if I have tattoos? A: Yes.
Q: Can I be an egg donor if I take recreational drugs?
A: Egg donors need to be free of any drug use for at least one year. Ad hoc use of cannabas is not encouraged if you are participating on an egg donor program.
Q: Can I be an egg donor if I have diabetes?
A: Yes. Hormone tests will be done. Fasting insulin will also be done and if hormone levels are not suitable, unfortunately the offer for egg donor help will not be able to proceed.
Q: Can I be an egg donor if I am depressed?
A: The nature of medication you are on will influence this decision.
Q: Can I be an egg donor if I am on oral contraceptive? A: Yes, there is no need to come off contraceptives. Only with the mirena loop will the loop need to be removed - This contains hormones which does not help egg donation.
Q: Can I become an egg donor if I am on the contraceptive injection?
A: First stop the injection and wait for your period to resume - only then can we do blood tests. The three month injection can sometimes take 9 - 12 months before it is completely out of an egg donor's system. Top fertility clinics have recommended that an egg donor have two normal menstrual cycles after the injection's effective date before she can donate her eggs. Accordingly it is advsed that a donor first have her normal periods before she proceed to register as an egg donor. This contraceptive injection applies to the Petogen/ Depo Provera injection. Normally a progesterone blood test will be done on day 21 of the second normal menstrual period to ensure that the egg donor has resumed ovulating. This will be a confirmation of her natural fertility having returned.
Q: Are blood tests required on any specific day?
A: Blood tests will be done on the same day as the initial appointment. Blood tests can only happen for ladies on the contraceptive injection once their cycle returns. The exact day on which blood tests will happen are dependent on the specific fertility clinic/ egg donation clinic.
Q: Can I be an egg donor if I am on elxtroxin?
A: A person taking eltroxin is attempting to regulate thyroid function. This is imperative for good egg development. It is therefore not ideal for egg donors to have a less than perfect thyroid function and egg donors taking eltroxin will unfortunately be declined from the egg donor program.
Q: Can I be an egg donor if I am pregnant? A: First have your baby, complete your breastfeeding and once your normal cycles resume, we can do the necessary tests for egg donation.
Q: Can I become an egg donor if I have been sterilsed or had my tubes tied? A: Yes, this is perfect.
Q: What other contraception can I use if I stop using my ordinary contraceptive? A: Condoms are considered the best contraceptive method.
Q: Why is there a cut off age of 34 for egg donors? A: Egg quality is deemed to deteriorate with age.
Q: Why do some fertility clinics require the egg donor to be aged 21?
A: Egg donation is a very mature and responsible giving deed. It is an elective process and egg donors are encouraged to enter the process with full understanding of the egg donor program.
Q: Will the child or children conceived through egg donation be able to contact the egg donor at a later stage? A: No. Egg donation is kept completely confidential.
